
Our policies are designed to ensure a safe, professional, and personalized experience for every client. Please review them carefully prior to scheduling your appointment.
I. General Services
• A Preliminary Consultation (Discovery Screening), provided as a complimentary virtual-only session, is required prior to your Initial Consultation.• All services provided are wellness-focused and are not intended to diagnose, treat, cure, or prevent any medical condition.• Clients are responsible for consulting their licensed healthcare provider regarding medical diagnoses, medications, urgent medical concerns, or treatment decisions.
II. Client Eligibility
• Clients must be 18 years of age or older to receive services.• A valid government-issued photo ID is required at the time of service.
III. Intake & Required Documentation
• Client registration begins at the time of booking as active client. All required intake forms, consents, and documentation must be completed in full prior to appointment confirmation.
• A comprehensive wellness assessment is required before initiating services and must be completed annually to maintain active client status. Clients are responsible for promptly reporting any changes to their health history or personal information, as such changes may require a follow-up assessment or, when clinically appropriate, a new comprehensive wellness assessment to establish an updated baseline before services may continue.
• Clients scheduling appointments near the 12-month mark may be required to complete a reassessment at the practitioner's discretion if updated clinical information is needed to provide safe and appropriate recommendations.
IV. Appointment Scheduling, Late, and Cancellation Policy
• Appointments may be scheduled up to 14 calendar days in advance, unless otherwise specified. Consultation fees are collected at the time of booking to reserve your appointment.
• Appointments begin promptly at the scheduled time. A 15-minute grace period is provided. Clients arriving more than 15 minutes after their scheduled appointment time will be considered a missed appointment and must reschedule.
• Appointments may be canceled up to 24 hours before the scheduled appointment through the Setmore booking confirmation email (the email containing your Booking ID).
• A $75 missed appointment fee applies to appointments that are missed or rescheduled with less than 24 hours’ notice. Cancellations made within 24 hours of the scheduled appointment may result in forfeiture of the consultation fee at the practice’s discretion. An unpaid balance in fees for missed appointments and less than 24 hours' notice
cancellations risk forfeiture.
• Professional fees are tailored to the services selected during your consultation, including customized laboratory testing, IV therapy, and other personalized offerings. A customized invoice will be provided prior to service. Payment plans may be extended at the practice’s discretion. We do not accept or bill insurance.
• Appointments are non-refundable, non-transferable, and reserved exclusively for the scheduled client. Forfeited consultation fees are not eligible for refund.
V. In-Home Services
• In-home services include professional travel time, preparation, and scheduling buffers to ensure service quality, safety, and continuity of care.
VI. Phlebotomy & Laboratory Services
• Phlebotomy and non-invasive clinical measurements and non-invasive assessments may be obtained as part of your nutrition physical assessment. Phlebotomy and non-invasive services require separate written consent at the time of service.
• Blood samples are collected by the practitioner and submitted to an independent CLIA-certified laboratory for processing and analysis.
• A Comprehensive Metabolic Panel (CMP) and Complete Blood Count (CBC) are required for all Initial Consultations and are included in the Initial Consultation fee. Clients may submit qualifying external laboratory results completed within the preceding 90 days for practitioner review.
VII. Supplement & OTC Guidance
• Dietary supplements and over-the-counter (OTC) products are purchased separately unless otherwise specified.
• Supplement and OTC guidance is provided for educational wellness purposes only.
• Clients should consult their licensed healthcare provider or pharmacist before starting any recommended dietary supplement or OTC product.
VIII. Injection Therapy & IV Protocol Guidance
• The Origin Method does not perform injection therapies or IV administrations. Any IV formulation or protocol guidance provided is strictly for educational and wellness purposes and does not constitute medical orders, prescriptions, or treatment authorizations.• IV and injection administrations are performed by independent, licensed medical providers who are solely responsible for their own intake, screening, informed consent, clinical decisions, administration, and outcomes. The Origin Method assumes no responsibility or liability for services provided by third-party medical providers.
IX. Telehealth Platform & Electronic Communication Consent
• The Origin Method uses secure electronic communication platforms, including Doxy.me our HIPAA compliant virtual suite, Delta Chat, Proton Mail, and text messaging, for wellness consultations, education, document sharing, and client communication.
• Electronic communication should not be used for medical emergencies or urgent medical concerns.
X. Availability & Service Limitations
• Services, therapies, and recommendations are subject to availability.• The Origin Method does not guarantee availability, outcomes, or individual results from any wellness service or recommendation.
XI. Data Use, Privacy, and Electronic Consent
• Electronic signatures are legally binding and carry the same authority as handwritten signatures. Signed documents remain in client permanent records and can be received at client’s request.
XII. Updates & Client Responsibility
• Terms & Conditions may be updated, revised, or modified at any time to reflect operational, legal, or Conditions prior to booking services.• Clients are encouraged to contact The Origin Method with any questions prior to scheduling.
