I. General Services
• A general consultation (discovery screening) is required prior to attending your initial consultation. Initial consultation bookings may be accepted; however, completion of the general consultation is required before the initial consultation can proceed.• All services provided are wellness-focused and are not intended to diagnose, treat, cure, or prevent any medical condition.• Clients are responsible for consulting their licensed healthcare provider regarding medical diagnoses, medications, urgent medical concerns, or treatment decisions.
II. Client Eligibility
• Clients must be 18 years of age or older to receive services.• A valid government-issued photo ID is required at the time of service.
III. Intake & Required Documentation
• All required intake forms, consents, and documentation must be completed in full prior to appointment confirmation.• Failure to complete required documentation may result in appointment cancellation, delay, or rescheduling at the practitioner’s discretion.• Annual Assessment Requirement: A comprehensive assessment is required prior to initiating services and must be completed annually to maintain active client status and continue receiving services.
IV. Appointment Scheduling, Late, and Cancellation Policy
• Appointments are scheduled no more than 14 calendar days in advance unless otherwise specified.• A 15-minute grace period is provided. If arrival exceeds this window, a late fee may be applied or the appointment may be rescheduled.• Missed, late, or cancelled appointments outside the stated policy window are non-refundable.• All booked appointments are non-refundable and non-transferable.
V. In-Home Services
• In-home services include professional travel time, preparation, and scheduling buffers to ensure service quality, safety, and continuity of care.
VI. Phlebotomy & Laboratory Services
• Phlebotomy services involve blood collection only and are not considered injection therapy.• Blood samples are collected by the practitioner and submitted to independent, CLIA-certified laboratories for processing and analysis.• Laboratory processing timelines, reference ranges, and reporting standards are determined solely by the laboratory.• Laboratory results are reviewed and shared for educational and wellness guidance purposes only.• Laboratory Requirements: A Comprehensive Metabolic Panel (CMP) and Complete Blood Count (CBC) are required for all Initial Consultations. Laboratory testing is included as part of the Initial Consultation fee. Clients may submit external CMP and CBC results completed within the previous 90 days for practitioner review. Qualifying external laboratory results may be accepted at the practitioner’s discretion; however, clients may still elect to complete the laboratory testing included with the Initial Consultation. If qualifying external laboratory results are not available, the included laboratory testing will be required prior to initiating services. The Initial Consultation fee remains the same regardless of whether external laboratory results are submitted or new laboratory testing is completed.
VII. Supplement & OTC Guidance
• Dietary supplements and any over-the-counter (OTC) products are purchased separately unless otherwise specified. When clinically appropriate, the practitioner may provide educational information regarding commonly available OTC products, including digestive support products. Any discussion of a specific OTC product is intended solely as a recommendation for the client to discuss its appropriateness with their primary healthcare provider or pharmacist and should not be interpreted as a prescription or directive to use the product.
• The Origin Method does not manufacture, distribute, sell, or receive compensation for dietary supplements or OTC products. Any products or brands discussed during consultation are provided solely as educational examples. Clients are free to purchase any product or brand that meets the practitioner’s recommended specifications.
• Clients are responsible for reviewing all product labels and for accurately disclosing all prescription medications, over-the-counter (OTC) products, dietary supplements, medical conditions, and known allergies before beginning any recommended wellness regimen.
• Clients should consult with their primary healthcare provider or pharmacist before starting any recommended dietary supplement or OTC product, particularly if they are pregnant, nursing, taking prescription medications, or managing a medical condition.
• Dietary supplement and OTC product information is provided for educational purposes to support general wellness and is not intended to diagnose, treat, cure, or prevent any disease.
• Individual responses to dietary supplements and OTC products vary. The Origin Method makes no representations, guarantees, or warranties regarding the safety, effectiveness, or outcomes of any product or recommendation.
VIII. Injection Therapy & IV Protocol Guidance
• The Origin Method does not perform injection therapies or IV administrations. Any IV formulation or protocol guidance provided is strictly for educational and wellness purposes and does not constitute medical orders, prescriptions, or treatment authorizations.• IV and injection administrations are performed by independent, licensed medical providers who are solely responsible for their own intake, screening, informed consent, clinical decisions, administration, and outcomes. The Origin Method assumes no responsibility or liability for services provided by third-party medical providers.
IX. Telehealth Platform & Electronic Communication Consent
• The Origin Method uses secure electronic communication platforms, including but not limited to telehealth-enabled platforms (such as Doxy.me), encrypted email services (such as Proton Mail), and text messaging via the practice mobile number, for wellness consultations, education, and client communication. By participating, you consent to communication through these electronic methods. These services are wellness-based and do not constitute telemedicine, medical care, diagnosis, or treatment.• You acknowledge and accept the inherent risks of electronic communication, including delays, technical failures, or privacy limitations beyond the control of The Origin Method.• You understand that The Origin Method provides non-medical wellness services only, and you are responsible for seeking care from a licensed medical provider for any medical concerns.
X. Availability & Service Limitations
• Services, therapies, and recommendations are subject to availability.• The Origin Method does not guarantee availability, outcomes, or individual results from any wellness service or recommendation.
XI. Data Use, Privacy, and Electronic Consent
• Client information is handled in accordance with applicable privacy and security standards.• Electronic signatures are legally binding and carry the same authority as handwritten signatures.• Signed consents and documentation apply to current and future services and are securely maintained in the client file.
XII. Updates & Client Responsibility
• Terms & Conditions may be updated, revised, or modified at any time to reflect operational, legal, or regulatory changes.• Clients are responsible for reviewing the current Terms & Conditions prior to booking services.• Clients are encouraged to contact The Origin Method with any questions prior to scheduling.
